JOB SUMMARY
This role will involve day-to-day management of our nurse practitioner network for Annual Health Assessments.
ESSENTIAL DUTIES AND RESPONSIBILITIES of Clinical Coordinator
- Manage team of Nurse Practitioners that will perform in-home and telehealth assessments on Medicare Advantage and ACA members
- Provide Feedback to Nurse Practitioner (NP) based on all findings including quality of documentation and clinical decision making
- Assist in pilots for new process ideas to improve the NP and member experience
- Assist in innovating and training on ways to improve recapture of known diagnoses during assessments
- Collaboration with Coding Account Managers (Risk Adj) and PHA Training Team on items for new NP training or retraining
- Participate in PHA quality taskforce team in performing GAP closure efforts Outreach to assigned Provider Network
- Obtain NP availability – based on company’s needs and membership
- Provide membership numbers/details in provider’s area
- Facilitate completion and tracking for outstanding trainings when non-responsive to training team
- Reduction of provider cancellation for In-home or Telehealth appointments
- Coaching and identification of corrective action for Complaints from client/patients
Requirements
- Minimum 1-3 years of Health Assessment Experience or direct patience care
- Minimum of 1-3 years’ experience as team leader, supervisor and/or trainer in a clinical setting
- Knowledge of Power Point and Excel
- Good language/verbal communication skills
- Good written communication skills
- Current NP or RN license in at least one state
