The Hospice Licensed Practical Nurse (LPN) works closely with RN Case Manager(s) to provide holistic patient-centered care, quality care end of life. As part of a collaborative interdisciplinary team, the Hospice LPN provides compassionate supportive care to our Hospice patients, as well as their caregivers, and families.

Highlights for a Hospice Licensed Practical Nurse (LPN):
- One on one patient care visits in the home or care facility ALF/LTC
- Timely completion of required documentation, adhering to agency policy
- Attend and participate in weekly IDT meetings
- Manage comfort measures to ensure quality of life that is based upon the patient’s goals
- Follow patient care plans, and orders
- Coordinate with RN Case Manager, attending providers and other IDT team members
- Engage in respectful communication (both verbal and written) with team members, patients, family members, and the community
- Adhere to agency wide policies
- Other duties as assigned
Qualifications for Homecare Registered Nurse (RN) Case Manager:
- Current NH LPN license
- Minimum 2-3 years direct care experience
- End of life or Hospice experience preferred
- Excellent communication skills necessary
- Valid driver's license
- Automobile insurance
- Proof of Flu vaccination
- Physical within the last year
- Two-step TB test
