Hospice Licensed Practical Nurse (LPN)

The Hospice Licensed Practical Nurse (LPN) works closely with RN Case Manager(s) to provide holistic patient-centered care, quality care end of life. As part of a collaborative interdisciplinary team, the Hospice LPN provides compassionate supportive care to our Hospice patients, as well as their caregivers, and families.

Regular Full-time, part-time, per diem

Summary:

• Hospice LPN
• Full Time: Monday - Friday, Days
• Jefferson City, MO and surrounding areas
• Pay Range: $50,000 - $70,000

The LPN will be responsible for delivering care to hospice patients in accordance with the care plan established by the interdisciplinary team.
