Part-time
Full Job Description
At Jaan Health, we believe the delivery of healthcare needs to change from an episodic, reactive process that deals with patient needs as they arise, to one that proactively manages patient health to prevent life-changing problems before they happen - especially for those patients with chronic conditions.
That’s why we’ve created Phamily, a solution that makes it possible for healthcare providers to proactively follow up and check-in on all of their patients with chronic disease, in between office visits and at a large scale.
The result - patients get better quality of care and are less likely to go to the Hospital/ER, providers and teams get paid for the work they do to support patients in between office visits, and payors see a significant reduction in total cost of care. It’s a rare win-win-win in healthcare where every party involved receives some benefit. Now, due to COVID-19, the need for this type of virtual care management is critical for keeping patients healthy and safe while they are at home.
We've helped tens of thousands of patients since our founding. Get in on the ground level of a high-growth company that is revolutionizing healthcare delivery.
This role is part time up to 30 hours a week Monday through Friday, normal business hours.
