As a Dementia Care Manager at Cary, you will bridge the gap between innovative technology and compassionate care. Your expertise will help patients and their families navigate the complexities of dementia while leveraging Cary’s cutting-edge AI tools. You will provide care coordination, education, and personalized care planning for individuals with dementia and their families.
Key Responsibilities:
- Cognitive Screening: Administer brief cognitive screenings to help physicians detect cognitive impairment in patients.
- Cognitive Testing: Assist in the administration and review of standardized cognitive performance evaluations, such as the Montreal Cognitive Assessment (MoCA), and conduct functional assessments for individuals with memory loss.
- Assessment and Care Planning:
- Perform virtual/telephone psychosocial assessments and develop care plans, including:
- Using standardized assessments such as the Functional Assessment Staging Test (FAST) and Clinical Dementia Rating (CDR).
- Administering medication reconciliation reviews.
- Evaluating behavioral symptoms, including depression and anxiety, using standardized screening tools.
- Conducting safety evaluations.
- Identifying social supports and assessing caregiver capability to deliver care.
- Care Coordination: Act as the primary point of contact for patients and caregivers, providing consistent communication to support care navigation and seamless care delivery.
- Personalized Care Planning: Conduct comprehensive assessments using AI insights to develop, write, and update tailored care plans.
- Patient & Family Support: Educate caregivers on dementia progression, care strategies, and available resources. Address social, emotional, and behavioral challenges with empathy.
- Medication and Health Monitoring: Support medication adherence, monitor health changes, and ensure timely updates to healthcare providers to enhance safety and well-being.
- Resource Navigation: Connect families with financial, legal, and social support systems to aid long-term planning.
- AI Integration: Collaborate with Cary’s AI tools to enhance care delivery, streamline processes, and improve outcomes.
Requirements
- Education: Bachelor’s degree in healthcare (e.g., Nursing, Social Work, Gerontology).
- Experience:
- Preferred: 1-3 years of experience with psychological/neuropsychological testing.
- Required: 1-3 years of experience working with dementia patients or geriatrics.
- Skills:
- Strong knowledge of dementia and associated care needs.
- Excellent communication and organizational abilities.
- Proficiency with electronic health records (EHRs) and care management software.
- Mindset:
- Openness to technology integration and willingness to adapt to innovative methods in care management.
- Good to Have:
- Master’s degree in Social Work (MSW), Nursing (MSN), or a related field.
- Experience in case management, elder care, or chronic disease management.
- Certification in Care Management (CCM), Dementia Practitioner (CDP), or Geriatric Care Management (CMC).
