Houston Behavioral Healthcare Hospital (HBHH) currently has an opening for a Outpatient Navigator fulltime (Monday-Friday, 8am-4pm). The Patient Clinical Navigator is responsible for navigating newly admitted patients to HBHH from community partners, transfer hospitals and surrounding communities to receive timely, patient-centered care at HBHH Outpatient Department. The Patient Navigator will be responsible for co-response, crisis intervention, case follow-up, referrals and connection to services, as well community outreach, engagement, training, and relationship building activities to increase behavioral health and resiliency, substance use awareness, and suicide awareness and prevention.
Successful performance in this position requires well developed interviewing skills as well as solid knowledge of outpatient practice services, procedures, and philosophy of patient care. This position also requires excellent organizational and interpersonal communication skills, basic computer proficiency, ability to manage multiple tasks simultaneously, and capacity for effectively managing stressful interpersonal interactions. The HBHH Clinical Patient Navigator participates in staff training as directed and assumes other duties as assigned by the Director of Outpatient Services.
Requirements
Education: Masters Degree in Social Work or Counseling required
Requirements:
LCSW, LMSW, LPC, LPC-A; Education, training, experience, and clinical supervision may also be observed.
CPR (Upon completion in Orientation or have a current CPR) and Hospital De-escalation Certification (Upon completion in Orientation)
Experience:
1 to 2 years experience working with Adults, Adolescents, caregivers and families with addiction and mental health needs.
Understanding of challenges faced by patients entering behavioral health treatment, including:
*Mental health disorders
*Dual diagnosis disorders
*Substance use disorders
*Ability to connect families with resources and evaluate provided services.
Experience conducting clinical interviews and coordinating/contributing to case management/social work.
Preferred experience in a healthcare.
Proficiency in computers, spreadsheets, and word processing.
Experience interacting with a diverse population while employing a strong customer service philosophy.
2 to 5 years of experience or a combination of education and experience preferred
