Outpatient Navigator (Full-time)

Houston Behavioral Healthcare Hospital (HBHH) currently has an opening for a Outpatient Navigator (fulltime). The Patient Navigator is responsible for navigating newly admitted patients to HBHH from community partners, transfer hospitals and surrounding communities to receive timely, patient-centered care at HBHH Outpatient Department. The Patient Navigator will be responsible for co-response, crisis intervention, case follow-up, referrals and connection to services, as well community outreach, engagement, training, and relationship building activities to increase behavioral health and resiliency, substance use awareness, and suicide awareness and prevention.
