Elder Care Homecare is seeking a dynamic Social Work Care Manager to join our company's team. We are a private pay homecare agency servicing New York and Connecticut, with additional expansion planned within the next year.
Elder Care has been in operation for over seven years, however our growth has dramatically increased over the last two. This role is based in midtown Manhattan and reports to the Director of Operations for our NYC office.
As a Social Work Care Manager, you will be responsible for overseeing overall case management within the client's home. That includes coaching direct care workers, recommending changes within the care given, periodic visits in the home, and liaising with family members and the company. The ideal candidate will have a strong background in homecare/geriatric care management, excellent interpersonal skills, a desire to lead, and dedication to caring for the elderly. Responsibilities:
- Working alongside the Director of Operations for our NYC office and others within your team
- Overseeing Clientele that is assigned to your care
- Creating and maintaining relationships with our Caregivers and Nurses caring for your clients
- Assisting with, implementing, and updating the plan of care for the client, ensuring Caregiver is aware of changes
- Assisting Coordination team with personality match between Client and Caregiver
- Home visits to cases that need staff mediation or supervision
- Onboarding prospect clients
- Monitoring discharge from Hospital, SNF or Rehab
- Some filing administrative paperwork in accordance with our policy and NYS DOH
- Occasionally hosting virtual enrichment classes for our Caregivers
Requirements
- Social Work degree or substantial related experience
- 2+ years of Social Work/Care Management experience
- Experience working with the geriatric community
- Comfortable in a leadership position
- Self starter who can work autonomously and stay organized
- Enjoys collaborating with a team with strong communication skills - able to communicate with colleagues, clients and caregivers
- People person
- Must be comfortable with conflict resolution and solution-oriented
