Social Work Care Manager

Elder Care Homecare is seeking a dynamic Social Work Care Manager to join our company's team. We are a private pay homecare agency servicing New York and Connecticut, with additional expansion planned within the next year.

Elder Care has been in operation for over seven years, however our growth has dramatically increased over the last two. This role is based in midtown Manhattan and reports to the Director of Operations for our NYC office.

As a Social Work Care Manager, you will be responsible for overseeing overall case management within the client's home. That includes coaching direct care workers, recommending changes within the care given, periodic visits in the home, and liaising with family members and the company.
