LCSW/LMSW Case Manager

We are seeking a licensed clinical social worker (LCSW) or licensed master social worker (LMSW) to join our team as a Case Manager. The ideal candidate will be responsible for providing comprehensive case management services to individuals and families in need.

Responsibilities:

- Conduct initial assessments to determine clients' needs and develop individualized care plans

- Provide counseling and support services to clients dealing with mental health and/or substance abuse issues

- Coordinate with external agencies and service providers to ensure clients receive the necessary support and resources

- Monitor client progress and make appropriate referrals as needed

- Maintain accurate and up-to-date case records and documentation

- Collaborate with a multidisciplinary team to ensure the overall well-being of clients

Qualifications:

- Master's degree in social work

- LCSW or LMSW license in the state of employment

- Experience in case management, preferably in a healthcare or social services setting

- Strong communication and interpersonal skills

- Ability to work effectively in a fast-paced and dynamic environment

- Understanding of and commitment to ethical practices and confidentiality
