Duties Highlights for a Licensed Practical Nurse (LPN):
- Provide skilled nursing care to patients
- Provide meaningful education to patients
- Support the RN Case Manager
- Execute tailored Care Plans
Qualifications for Homecare Registered Nurse (RN) Case Manager:
- Proof of Flu vaccination
- Current NH LPN license
- Home Care experience beneficial, although we will train the right individual
- Automobile insurance
- Physical within the last year
- Two-step TB test
