Hospice Clinical Manager

As the Hospice Clinical Manager, you will report directly to the Chief Clinical Officer and serve as a key member of the Hospice management team by leading a full team of multidisciplinary, healthcare professionals. Primary work will include the supervision, management, coordination and provision of quality patient care while demonstrating full understanding of the Hospice COPs and maintaining the highest standards of care. This position requires you to have a current NH RN License.

Responsibilities of the Hospice Clinical Manager:

• Manages and coordinates provision of quality hospice services from admission to discharge
• Coordinates IDT meetings and promotes communication of information relevant to the patient care process to ensure quality of care and coordination of services
• Oversees daily operations and schedules
• Oversees development and updating of patient care plans
• Assists with and coordinates training of appropriate staff, with special attention to case management skills, and assists with performance evaluations of team members
• Participates in agency quality assurance activities designed to improve quality and continuity of patient care
• Reviews data and assists with education of hospice staff to achieve high quality care and documentation
• Supports, where appropriate, the hospice referral process and the smooth transition of patients to LSRVNA hospice services from referral sources

Qualifications of the Hospice Clinical Manager:

• Current NH RN license
• 1 – 2 years supervisory experience required
• 3 – 5 years hospice or home health experience preferred
• Excellent communication skills necessary (verbal and written)
