The Hospice and Palliative Care Coordinator is a professional registered nurse who is responsible for overseeing the coordination of care for new and existing Palliative and Hospice patients. Responsible for coordination of new patient admissions, and existing patients, daily scheduling, and collaboration with multidisciplinary team members.

Essential Job Functions/Responsibilities of a Hospice and Palliative Care Coordinator:
- Works closely with Hospice Clinical Manager to ensure coordination and quality of patient care delivery
- Reviews new patient referrals, collaborates with care team, ensures all necessary items are present, and coordinates DME, medications and scheduling of admission
- Assist with daily scheduling and as needed visit management
- Assists in coordinating patient care transitions for change in hospice level of care and/or transition between programs
- Responsible to oversee process for managing and maintaining new pending palliative referrals and palliative care case load
- Acts as point person for Hospice and Palliative care nurse practitioner
- Assists with Hospice patient triage
- Manages task lists in MatrixCare EMR
- Collaborates with all agency departments to ensure appropriate services for patients
- Communicates with palliative and hospice patients and families
- Acts as a resource to support field staff as needed
- Maintains current knowledge of Hospice and Palliative policies and practices
- Participates in interdisciplinary team meetings as needed
- Engages in respectful, professional communication
- Adheres to agency wide policies
- Other duties as assigned
Position Qualifications of a Hospice and Palliative Care Coordinator:
- Active NH Registered Nurse (RN) License
- Three (3) to five (5) years of health care experience
- Excellent communication and coordination skills
- Home health or Hospice experience preferred
- Automobile insurance
- Physical within the last year
- Two-step TB test
- Proof of Flu vaccination
