Seasoned Recruitment is currently seeking a dedicated and experienced Social Worker for the position of Case Manager. In this role, you will play a vital role in providing comprehensive support and assistance to individuals and families in need. Your main responsibility will be to assess clients' needs, develop individualized care plans, and coordinate services to ensure the best possible outcomes. Responsibilities
- Conduct comprehensive assessments of clients' needs and develop individualized care plans
- Oragnize and incorporate information from all available sources, including consumer self-reports, reports from family members, guardians, and Adult Foster Care providers, documented medical and treatment history, needs surveys, and assessments from other disciplines.
- Coordinate and facilitate services and resources to meet clients' needs
- Advocate for clients' rights and ensure they have access to necessary support and services
- Collaborate with a multidisciplinary team to provide holistic care
- Monitor and evaluate clients' progress and adjust care plans as needed
- Complete 25 hours of program and or position-specific training on an annual basis
- Maintain accurate and up-to-date documentation
Requirements
- A bachelor's or master's degree in social work (BSW/MSW).
- Prior experience as a Social Worker or Case Manager preferred
- Qualified Mental Health Professional credential within 120 days of hire is required
- Must have 1 year of experience (may include time spent in fieldwork or internship) treating or working with people who have been diagnosed with mental illness.
- Strong communication and interpersonal skills.
- Excellent problem-solving and decision-making abilities.
- Ability to work collaboratively in a team setting.
- Experience and knowledge of community resources and support networks.
- Valid driver's license and reliable transportation
