Reports to: Nursing Supervisor/Manager
Job Summary: The Registered Nurse (RN) will provide professional nursing care to patients in their homes, ensuring the highest level of comfort, safety, and recovery. The RN will collaborate with physicians and multidisciplinary team members, provide physical and psychological support to patients, friends, and families, and ensure continuity of care.
Key Responsibilities:
- Conduct comprehensive patient assessments to identify patient needs.
- Develop and implement individualized nursing care plans.
- Administer medications and treatments as prescribed by the physician.
- Monitor patient conditions and responses to treatments.
- Educate patients and their families on disease processes, medical treatments, and self-care techniques.
- Coordinate with physicians, other healthcare professionals, and home health aides to ensure comprehensive patient care.
- Maintain accurate, detailed, and timely records of nursing actions and patient conditions.
- Perform wound care, catheterizations, and other specialized nursing procedures.
- Ensure compliance with healthcare regulations and standards.
- Provide emotional support to patients and families.
- Participate in patient discharge planning and follow-up care.
- Conduct in-home safety assessments and make recommendations for home modifications.
- Supervise and guide licensed practical nurses (LPNs) and home health aides.
Qualifications:
- Current Registered Nurse (RN) license in the state of Michigan.
- Bachelor’s Degree in Nursing (BSN) preferred.
- Minimum of 1-2 years of clinical experience, preferably in home health care.
- CPR/BLS certification.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid driver’s license and reliable transportation.
Licensed Practical Nurse (LPN)
Position: Licensed Practical Nurse (Onsite Home Health Care)
Reports to: Registered Nurse/Nursing Supervisor
Job Summary: The Licensed Practical Nurse (LPN) will provide nursing care to patients in their homes under the direction of a Registered Nurse. The LPN will assist in ensuring patients receive quality care, support with daily activities, and maintain optimal health and comfort.
Key Responsibilities:
- Provide basic nursing care, including administering medications, monitoring vital signs, and dressing wounds.
- Assist with daily living activities, such as bathing, dressing, and grooming.
- Monitor and report changes in patient health status to the RN.
- Assist in developing and updating patient care plans.
- Educate patients and families on health maintenance and disease prevention.
- Document patient care and progress notes accurately and timely.
- Collaborate with the RN, physicians, and other healthcare professionals to ensure coordinated care.
- Assist with rehabilitation activities and exercises as prescribed.
- Perform basic laboratory tests and collect specimens.
- Ensure patient safety and comfort at all times.
- Maintain cleanliness and organization of patient care environment.
- Support patients and families emotionally and psychologically.
