Full Job Description
Position Summary: MD Disaster and Crisis is seeking a highly skilled and compassionate Bilingual Case Manager to join our team and work with ORR youth. As a Case Manager, you will provide comprehensive case management services to individuals and families who have been affected by disasters and crises.
Working closely with clients, you will assess their needs, develop individualized care plans, and connect them with appropriate resources and support services. You will also provide ongoing support, advocacy, and coordination of services to ensure clients receive the assistance they need to recover and rebuild their lives.
Requirements
Job Duties and Responsibilities include:
- Conducting comprehensive assessments to identify clients' needs and determine eligibility for services.
- Developing individualized care plans in collaboration with clients.
- Assisting clients in accessing and navigating community resources and support services.
- Providing ongoing support, guidance, and advocacy to clients.
- Coordinating and facilitating referrals to support services, such as mental health counseling, housing assistance, and financial resources.
- Collaborating with other professionals, agencies, and organizations to ensure coordinated and effective service delivery.
- Documenting clients' progress, services provided, and outcomes.
- Participating in case conferences and multidisciplinary team meetings to review client progress and adjust care plans as needed.
- Case Manager Qualifications and Skills
- Bachelor's degree in social work, psychology, or a related field; Master's degree preferred.
- Prior experience in case management or related field.
- Knowledge of community resources and support services.
- Strong assessment and problem-solving skills.
- Excellent communication and interpersonal skills.
- Ability to establish rapport and build trusting relationships with clients.
- Understanding of trauma-informed care principles.
- Ability to work independently and as part of a team.
- Proficiency in documentation and record-keeping.
