Full Job Description
Ther Mental Health Association is hiring immediately for a Program Coordinator. The Program Coordinator is responsible for the oversight of two homes that serve survivors of acquired brain injury or other major life changing event. They are responsible for the direction, culture and quality of care that is delivered throughout the sites to which they lead. They work in tandem with the Program Nurse and Program Clinician to provide the highest standard of care for all those served within the program. They will provide direct supervision to the Site Manager who provides on-site leadership at their site and Direct Support Specialists within the program. The Program Coordinator reports to the Director of Residential Services of New Way and is expected to maintain compliance with Federal, State, and Mental Health Association policies and regulations.
Position's Essential Functions
Supervisory
1. Oversight of the Site Manager and corresponding team to ensure that the highest level of care is provided to the people receiving care within the program. Remove barriers and problem solve around items that delay or
disrupt services.
2. Individual Service Plan development with the ISP team and oversight of implementation.
3. Facilitation of transitions into the home from skilled nursing facilities, including the management of the
Transitional Assistance plan necessary to move in.
4. Direct communication with DDS Service Coordinators for updates, HCICS reporting, and referral management.
5. Ensures all program deadlines are met through supervision, systems management and training.
6. Work to promote the missions of the division at a program level as well as the MHA core values of respect, integrity and compassion for all those receiving services.
7. Oversee hiring, onboarding/training, and disciplinary action within the program with the Vice President's approval.
8. Maintain licensing standards through audits, systems and training.
9. Act as a liaison between departments, vendors, state entities and families/guardians.
10. Act as a layer of support on the divisional on-call system to ensure the safe operation of services

Safety
• Maintain a safe workplace environment by identifying and addressing any safety hazards/issues, reporting all employee accidents to Human Resources and participant accidents to immediate supervisor, and following established MHA safety policies and procedures.
Requirements
- Demonstrated ability to work independently, manage time, prioritize tasks, take initiative, follow through, and work cooperatively to direct a team.
- Good oral, written, computer, and organizational skills.
- Demonstrated ability to adapt to the changing needs of the program participants. Strong interpersonal skills, patience, caring, and compassion.
- Must be able to pass and maintain certifications in medication administration (MAP), CPR, and first aid.
- Must have ability to facilitate and develop a supportive community in a diverse, multi-cultural residential setting.
