What You Will Do
As a Hereditary Hemorrhagic Telangiectasia (HHT) Advanced Practice Provider, you will be responsible to medically manage adult patients with HHT in collaboration with the IHTC physicians.
The Opportunity
- Triage/management of patient issues in the following areas: on-call, inpatient, and other outpatient setting. Management of HHT related complications, including epistaxis, iron deficiency anemia, and screening for arteriovenous malformations.
- Effective cooperation and communication with other HHT team members to ensure all patient needs are met, including phone triage.
- Patient management includes but not limited to health history, medication review, physical examination, ordering and interpretation of laboratory and radiology studies, development and implementation of a treatment plan, surgical care planning, education/counseling, ongoing monitoring and follow-up.
- Documentation in electronic medical record per policies of IHTC and St. Vincent Hospital.
- Identify at-risk family members of affected patients and coordinate screening/testing.
- Work with patients to successfully transition from the pediatric medical providers to adult providers at the appropriate age/developmental level.
- Perform procedures, as delegated with competency check off by the physician.
- Collect outcomes and data for applicable grant reporting.
- Authorized to administer Basic Life Support in accordance with current CPR standards.
Requirements
- Minimum Master’s degree
- Minimum 3 years of experience working with patients with a chronic disease
- Valid IN Nurse Practitioner or Physician Assistant (and, if applicable, PA-C) License
- Valid driver’s license and insured automobile required
