SCOPE OF POSITION:
The Home Coordinator provides and is responsible for the support, supervision, oversight and management in all aspects of Total Care Services Inc residential setting. The Home Coordinator must ensure the Persons rights are protected and advocated for always.
ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITES:
- Ensure the overall health, safety and wellbeing of all Persons living in the home.
- Do all shopping for food, clothing, and household supplies;
- Schedule all medical appointments for Persons assigned;
- Coordinate with the assigned Nurse, requirements for all medical appointments;
- Schedule transportation and or transport Persons when needed to day programs, medical appointments, meetings, etc;
- Ensures that the Persons make choices and actively engage in person centered planning;
- Oversee, supervise and evaluate Direct Support Professionals in a professional manner;
- Must visit Persons Day program at least once a month.
- Must call Persons Day program once a week.
- Find replacement staff, when regular staff calls off with the assistance of Home Coordinator Supervisor as needed;
- If on-call staff is not found, the Home Coordinator is required to work the shift;
- Coordinate, schedule, and plan for the Persons to participate in a variety of community activities of their choice;
- Supervise all planning, and preparation of meals in accordance to the Persons nutritional plan;
- Make sure the home is kept clean, dust free and sanitized always;
- Report any necessary repairs or maintenance to Residential Director/Home Coordinator Supervisor;
- Make sure all the Persons needs are met and not limited to toileting, bathing, eating, money management, menu planning, grocery shopping, dressing, and other areas in one’s daily living;
- Attend all mandatory meetings and trainings;
- Ensure that all documents are properly documented in the Person’s program planning record;
- Document all daily activities of Persons assigned in Therap;
- Monitor Direct Support Professional documentation in Therap;
- Complete all incident reports to be submitted to QIDP, Residential Director and Incident Management Coordinator as well as properly completing GER (General event report) in Therap;
- Follow all protocols for medical and/or behavioral emergencies as well as unwarranted infestations (i.e. bed bugs, roaches, ants, rodents etc.)
- Monitor time sheets daily to ensure all Direct Support Professionals time is accurate and properly documented.
- Ensure DSPs follow Covid-19 protocols when visitors enter the home
- Perform other duties as required and assigned.
COMMUNICATION:
- Ability to effectively communicate orally and in writing with internal and external business professionals.
- Communicate with all Persons supported and Direct Support Professionals appropriately.
- Ability to understand and follow all TCS/Residential Protocols
DECISION MAKING:
- Make safe and sound decisions in various work environments.
- Ability to make decisions about the safety and welfare of the persons supported.
- Ability to make decisions about the safety of the work environment.
WORKING CONDITIONS AND ENVIRONMENT:
- The work is performed in the field; however, some work requires office visitations.
- The work is partially sedentary and may require lifting, up to 50 pounds.
- Must have basic computer skills.
Requirements
- Must have at least 2 years supervisory experience supervising working with Persons with intellectual and developmental disabilities;
- Provide evidence of completion of at least High School Diploma or GED Certificate;
- Be at least 21 years of age;
- Complete a DOH Criminal Background check that is acceptable in accordance with D.C. government standards;
- Submit to random drug testing as needed;
- Obtain Trained Medication Employee (TME) certification and renew annually;
- Compete CPR and First Aid training in addition to all other trainings required by DDS;
- Must be able to communicate with Persons;
- Must be accessible to Persons;
- Must be able to recognize emergency and be knowledgeable about emergency procedures;
- Must be able to read and write the English language;
- Must have basic computer skills
- Must be a US citizen or alien who is lawfully authorized to work in the US; and
- Be free of communicable disease as confirmed by an annual PPD skin test/chest x-ray or documentation from a physician stating that the person is free from communicable disease.
- Submit an annual physical examination certifying that he/she is in good health and fit to perform the job responsibilities relevant to the position;
- Recommended to have a vehicle and a valid driver’s license.
- Must be able to provide evidence of COVID-19 vaccination and obtained the booster per DDS guidelines
