Hospice Admissions RN

The Hospice Admissions Registered Nurse, at the request of the Intake Department, provides nursing services to patients as ordered by the physician. The nurse is a specialist in assessing the patient’s condition and developing a plan of care to improve, monitor, and document the course of treatment prescribed by the physician for the patient’s diagnosis. The plan of care is developed to reduce symptoms, educate the patient, gain patient’s compliance to the prescribed plan of care, and assist the patient in becoming independent with self care when capable, based on the individual needs of the patient. Additionally, the nurse communicates with all members of the care team to insure proper coordination of care.

Phoenix Hospice Admissions RN assess and determines hospice appropriateness of patients seeking end of life care. The admission RN should possess strong skills in physical assessment, diagnostic interpretation, communication, and multidisciplinary approach that encompasses the physical, but also the spiritual and psychosocial needs of the patient and their family. Admission nurses work closely with our intake coordinators, hospice consultants and medical directors to ensure a smooth transition from curative care to hospice care.
