The Admissions Registered Nurse Home Health, at the request of the Intake Department, provides nursing services to patients as ordered by the physician. The nurse is a specialist in assessing the patient’s condition and developing a plan of care to improve, monitor, and document the course of treatment prescribed by the physician for the patient’s diagnosis. The plan of care is developed to reduce symptoms, educate the patient, gain patient’s compliance to the prescribed plan of care, and assist the patient in becoming independent with self care when capable, based on the individual needs of the patient.
