Case Manager Supervisor (Santa Rosa) in California

DESCRIPTION OF POSITION:

In collaboration with multidisciplinary team Outpatient therapists, provides quality case management and discharge planning services to all patients and their families. Serves as a member of interdisciplinary team supporting the organization's treatment program and philosophy, and assures the deliverance of quality treatment to patients and their families.

Lead Responsibilities:

• Maintain discharge reference tool to improve time management for staff
• Assist case managers with challenging cases on all units
• Report daily long lengths of Stay and assist DSS with reports to leadership
• Step in for treatment team when staff are absent and ensure all requirements for treatment team are met.
• Reports daily and biweekly discharge calendar updates.
