Acentra seeks a Service Support Facilitator to join our growing team. 

Job Summary:

As a Service Support Facilitator, you will be the initial point of contact for community members in need, ensuring that every child receives the highest quality behavioral health services. In this crucial role, you will engage with callers both over the phone and in person, using your clinical interviewing and assessment skills to gather vital information. This position offers a unique opportunity to significantly contribute to the delivery of exceptional behavioral health services for children, making a profound impact on the lives of individuals within our communities.

Your mission is to identify and recommend the most appropriate level of care, greatly enhancing the well-being of children and families. At Acentra Health, you will play an integral role in our mission to deliver outstanding services and foster lasting partnerships.

Job Responsibilities:

  • Conduct comprehensive member assessments within specified timelines, ensuring meticulous documentation throughout the process.
  • Accurately and promptly input assessment data into the web system.
  • Participate in inter-rater activities to ensure consistency in the administration of assessment instruments and adherence to protocols.
  • Provide training to provider agencies and other stakeholders as needed or assigned.
  • Manage special projects as assigned, demonstrating adaptability and a proactive approach.

Required Qualifications/Experience:

  • A minimum of 2+ years of hands-on experience working with children receiving mental health services, with a strong understanding of Child Welfare, Behavioral Health, and Medicaid policies and procedures.
  • Bachelor’s Degree in a related healthcare field.
  • Proficiency in person-centered planning fundamentals.
  • Advanced computer skills, including the ability to navigate multiple databases and software applications across various screens.
  • Strong proficiency in Microsoft Office, along with excellent internet/web navigation skills.
  • Demonstrated ability to maintain confidentiality standards and ensure HIPAA compliance when evaluating all relevant issues.

Compensation

The compensation for this role is $57,200 to $58,000 base salary.

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
