Acentra seeks a RN Clinical Assessor (Hybrid, West Virginia) to join our growing team.
Job Summary:
Join our team as an RN dedicated to the West Virginia Aged and Disabled Waiver (ADW) Pre-Admission Screen (PAS) programs. As an ADW RN, you'll play a pivotal role in educating and assessing program participants to determine their medical eligibility. This vital position combines healthcare expertise with compassion and dedication. This role offers a meaningful opportunity to make a difference in the lives of individuals within the community.
*Position is hybrid. Field assessments will primarily be in Braxton, Lewis, Upshur, and Gilmer counties of West Virginia. The ideal candidate will reside in one of these counties.
Job Responsibilities:
- Conduct thorough ADW assessments, primarily in the field, adhering to set timelines.
- Efficiently schedule and document ADW assessments within prescribed periods.
- Contribute to maintaining consistency in evaluations through participation in inter-rater reliability activities.
- Engage in denial hearings when necessary, advocating for program participants.
- Collaborate with physicians and facilities to verify medical information crucial to assessments.
- Embrace a flexible approach to duties and responsibilities as required.
- Note: Additional duties may be assigned by management.

Requirements
Required Qualifications/Experience:
- Must possess a valid and active West Virginia Registered Nurse license while residing in the state of West Virginia.
- Hold a Nursing degree (preference for a Bachelor’s degree).
- Exhibit comprehensive knowledge of WV Aged and Disabled Waiver policies and procedures, along with a willingness to conduct assessments in homes and facilities.
- Show proficiency in accurately assessing medical conditions, symptoms, and functional abilities.
- Be capable of independent remote work from a home office and working in the field, conducting face to face assessments.
- Must possess strong computer skills and be proficient in navigating through web-based applications, databases, programs, and multiple screens.
- Demonstrated proficiency in the Microsoft Office Suite coupled with strong experience with data entry.
Preferred Qualifications/Experience:
- 2+ years of experience in working with the aging population or individuals with disabilities. Previous exposure to the WV Aged and Disabled Waiver program and CareConnection© is highly desirable.

Compensation
The pay range for this position is $60,000-$70,000.
Based on our compensation philosophy, an applicant’s placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
