Requirements

Job Responsibilities:

• Assess patient conditions and collaborate with the healthcare team to develop and implement individualized care plans.
• Administer medications, treatments, and therapies as ordered by physicians.
• Monitor and document patient progress and respond to any changes in condition.
• Assist patients with activities of daily living and provide emotional support.
• Adhere to all facility policies, procedures, and regulatory requirements.
• Collaborate with patients, families, and caregivers to provide education and support.
• Maintain accurate and detailed medical records.

Job Requirements:

• Valid Registered Nurse (RN) license in the state of practice.
• Minimum of 2 years of experience in subacute care or a related field.
• Current BLS certification.
• Excellent communication and interpersonal skills.
• Strong clinical assessment and critical thinking abilities.
• Ability to work well in a team environment.
• Attention to detail and strong organizational skills.
