About the Role:

The case manager works under the supervision of the Senior Case Manager and assists low-income working families who have experienced homelessness and those at risk of homelessness. Case manager’s primary responsibilities are to the Family PASS Self-Sufficiency Program, which provides long-term intensive case management for up to three years and works with county resources to help clients attain the education and skills training needed to become economically self-sufficient with a focus on increased earnings through job training and additional education. The case manager supports a caseload of 15-20 households. Case Managers are to implement services with a trauma-informed, strengths-based, and client-centered approach while assisting households to work toward self-sufficiency. The case manager works closely with their supervisor to accomplish program goals and priorities in accordance with agency philosophy and values, and best practices for homelessness and survivors of domestic violence.

Requirements

Duties and Responsibilities:  

 

  • Interviewing and screening clients for eligibility for Family PASS’s Self-Sufficiency Program.
  • Assessment of client strengths and needs. Complete ongoing biopsychosocial-spiritual assessment to determine client strengths and needs and to ensure stability.
  • Develop and/or revises individualized client-centered case plan goals and action plans.
  • Coordination of linkage and referral to community resources to include services such as education/training, career counseling, mental health services, financial literacy, legal services, childcare, medical/dental services, etc.
  • Using a holistic approach consult with supervisors, and/or other programs to help families overcome barriers to meeting goals and objectives.
  • Advocate for client needs to ensure housing and financial stability.
  • Plan, schedule, coordinate, and facilitate meetings with clients on an ongoing and as needed basis.
  • Be prepared for and actively participate in staff meetings, training, and one on one supervisions and any other meetings determined necessary.
  • Creating, tracking, and maintaining client case files in accordance with agency and best practice standards.
  • Timely documentation of client contacts and case management activities in relevant databases and client files.
  • Ensure client data is entered into client lists and other relevant databases in an accurate and timely manner.
  • Perform other duties deemed necessary to support the program and agency. 

 

About You:

Required:

  • A bachelor's degree in social work/human services, related field, or commensurate experience 
  • 1+ years of case management/ direct services 
  • Strong written and oral communication skills 
  • Ability to prioritize competing priorities and make sound judgements 
  • Ability to complete tasks while navigating frequent interruptions 
  • Ability to deliver creative, resourceful solutions to unique challenges 
  • Strong organizational and time management skills 
  • Valid Driver’s License, personal reliable transportation, up to 50% local travel required for appointments
  • Ability to Drive a 15-passenger van and/or minivan

Preferred:

●       Foreign Language fluency in a language other than English (ex. Spanish, Farsi, Arabic or Amharic).

●       Experience with providing services to clients experiencing domestic violence, homelessness, mental health, or substance abuse issues.

 

Physical Requirements:

●       Annual TB Test is required.

●       Ability to sit or stand for long periods.

●       Ability to lift items weighing 10-20 pounds.
