The Road Home was founded in 1923 and has been a leader in the endeavor to end homelessness for over 100 years. We provide low-barrier emergency shelters, supportive services, and housing-first-minded rental assistance that helps individuals and families step out of homelessness and back into the community. We are seeking compassionate and empathetic individuals who are interested in making a difference in the lives of others and their community.
Job Summary
The Housing First for Individuals Program (HiFI) is a collective of case managers that provide support and services to households without children who have experienced chronic homelessness. The HiFI Case Manager works with program participants towards finding and maintaining housing within Salt Lake County while also creating and fostering community connections for participants when additional, specialized supports are needed. Individuals are referred to the HiFI team through a standardized community process involving multiple partner agencies and case managers are expected to be able to interact consistently and professionally with these partners at all times.
HiFI Case Managers provide support and services to program participants using a person-centered, housing first and trauma-informed approach. We promote and provide harm reduction services as needed and we are seeking individuals who can engage in open, honest and non-judgmental conversations around this.

*This is an in-person position that requires travel between various resource centers and shelters. There may be flexibility for occasional remote work with work plan approval from the HiFi Supervisor*

Job Duties
- Provide direct case management services to program participants. This includes crisis intervention, program assessments, goal setting, problem solving, agency and community resources utilization, and network building.
- Provide outreach services to clients in a variety of settings including their homes, shelters and outdoors when needed.Provide outreach services to clients in various settings including their homes, community resource centers and outdoors when needed.
- Keep accurate records of services provided to program participants and be able to submit all information in a timely manner.
- Attend regular meetings including a weekly clinical case staffing and a weekly one on one meeting with supervisor for support.
- Use agency vehicles to provide transportation for guests.
- Other duties as assigned.

Requirements
Education and Experience
- Experience working with diverse populations preferred.
- Must have either lived/worked/volunteer experience or a degree in a related field.

Physical and Equipment Requirements
- Ability to assist with light duty moving (food boxes)
- Ability to stand and walk for at least an hour at a time*
- Ability and license to drive a vehicle*
