Texas Health Action is currently seeking a Non-Medical Case Manager (NMCM) to serve our Kind Clinic locations in Austin. This position will work closely with Ryan White clients receiving sexual health services to coordinate a range of client-centered services linking them to healthcare, psychosocial services and support, and/or other services as needed. The NMCM focuses on improving access to and retention in needed core medical and support services. The NMCM also provides coordination, guidance, and assistance in accessing medical, social, community, legal, financial, employment, vocational, and/or other needed services.
What you would do:
- Conduct initial comprehensive non-medical assessment of clients’ needs and support systems.
- Develop a comprehensive, individualized service plan and coordination of the services required for implementation of plans for clients.
- Coordinate access to medically appropriate levels of health and support services and continuity of care.
- Assist clients in accessing routine medical care for HIV.
- Assist clients in applying for Social Security benefits and other types of entitlement programs.
- Educate client on the availability of all internal and external resources, programs, and services that are applicable to care
- Provide HIV risk reduction education and prevention.
- Ensure client’s compliance with medications, providing education and support as necessary.
- Provide ongoing non-medical assessment of the client’s and other key family members’ needs and personal support systems.
- Ensure that all eligible funding sources are evaluated before accessing Ryan White (RW) funds.
- Maintain assigned client’s medical and service record files and be responsible for file’s completeness.
- Abide by current RW Standards of Care, including RW eligibility documentation.
- Perform other duties as assigned.
Requirements
Minimum Qualifications
● 2+ years of experience in a healthcare delivery system including care navigation, advocacy, and/or case management
● 2+ years of experience in working with diverse populations, including LGBTQIA+ and communities of color
● Demonstrated experience taking initiative and to exercising independent judgment, decision-making, and problem-solving expertise
● Proficient using computer software applications, such as Windows and Microsoft
● Strong written and verbal communication abilities
Preferred Qualifications
● Bachelor's in Social Work from a school accredited by the Counsel of Social Work Education
● 2+ years of direct experience with HIV counseling and testing, case management and/or HIV programming
● Familiarity and experience using a trauma-informed approach to working with individuals
● Experience and comfort in issues related to substance use, harm reduction, homelessness, mental health, and LGBTQIA+ communities
● Ability to work harmoniously with diverse groups of individuals
● Experience working with an ethnically, culturally and racially diverse work staff
● Ability to deal effectively with stress relating to the changing organizational, personnel, and healthcare environment
Compensation
Based on job duties and requirements, this position is placed in salary band 40 with an hourly rate starting at $20.80 per hour non-exempt. Initial placemen will be based on hired candidate's qualifications and relevant experience.
