Abundant Hospice is seeking a dedicated Clinical Pharmacist to join our team, delivering exceptional pharmaceutical care tailored to meet the unique needs of our hospice patients. In this crucial role, you will collaborate closely with our interdisciplinary team to ensure that medication therapy is both safe and effective, particularly in managing pain and other symptoms associated with life-limiting illnesses. Your expertise will be invaluable in providing comprehensive medication reviews, developing individualized care plans, and educating both patients and their families about medication management. You will be responsible for optimizing pharmacotherapy, conducting medication reconciliations, and monitoring for potential drug interactions. As a Clinical Pharmacist at Abundant Hospice, your contribution will extend beyond medication management; you will provide compassionate support and guidance during one of life’s most challenging times. You will also stay up-to-date with the latest therapeutic practices to provide evidence-based recommendations to enhance patient outcomes. Responsibilities

  • Conduct comprehensive medication reviews for hospice patients.
  • Develop and implement individualized pharmacotherapy plans.
  • Monitor patient medications for efficacy, safety, and adherence.
  • Collaborate with healthcare professionals to optimize medication therapy.
  • Educate patients and families on medications, side effects, and management.
  • Perform medication reconciliations during patient admissions and discharges.
  • Stay current with pharmacotherapy guidelines and advancements in hospice care.

Requirements

  • PharmD degree from an accredited institution.
  • Active pharmacist license in the state of practice.
  • Completion of a pharmacy residency or equivalent experience preferred.
  • Experience in hospice or palliative care settings is a plus.
  • Strong knowledge of pharmacotherapy and medication management.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a collaborative, multidisciplinary team environment.
