Job Responsibilities:

Job Summary:

The Care/Case Management (CM) Lead is responsible for working in collaboration with Clinical and Contract Leadership on the development and training of care management staff, job aides and policies, and contributing to guidelines for systems to effectively operate a comprehensive Care/Case Management Program. The CM Lead also provides direct CM services to an assigned group of beneficiaries, while supporting the overall team and assisting with quality audits and client priorities.

Job Responsibilities

· Assessing, planning, implementing, monitoring, and evaluating actions required to meet the client’s beneficiary needs. Integrates and coordinates services using continuous quality improvement tools such as case and call audits for the CM team.

· Use of Assessments, any additional CM resources available, and the Atrezzo or client-based system.

· Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered goal planning by developing individual care plans.

· Assist with case assignments for the CM Team in collaboration with the Supervisors

· Develop effective working relations and collaborate with medical team throughout the entire case management process.

· Serve as a primary subject matter expert for case management processes

· Serve as a mentor and training support to the CM team

· Assist Supervisors with monthly quality audits for the CM team and communicate findings to the Supervisors

· Assist with the development or updating of policies, procedures, and job aids

· Collaboration on projects which are of priority to our client

Requirements

Required Qualifications/Experience

  • Active, unrestricted compact RN licensure
  • · Supervisory or Management experience; Preferred
  • · Minimum of 5 years’ experience in Case Management
  • · Certification in Case Management required (will consider if in progress)
  • · BSN; Preferred
  • · Effective and compassionate communication skills
  • · Excellent organizational and time management skills
  • · Problem solving skills and ability to multi-task
  • · Strong computer skills, knowledge and navigational skills
  • · Must work a full time, M - F
