We are seeking a Supported Living Advocate to support our Vista Care operations in South Dakota. 

 

JOB SUMMARY

The Supported Living Advocate, under the direction of the Residential Manager, assists with the coordination of individuals’ plans within their supported apartment and through community living. The Supported Living Advocate works to assist the individuals in achieving persona; goals in accordance with Vista Care Mission, Vision, and Values.

RESPONSIBLE TO

The Supported Living Advocate reports to the Residential Manager

RESPONSIBILITIES

  • Under direction of the Residential Manager, completes assessments with the individuals as requested or required. Reviews ICAP when completed by the case manager. 
  • Coordinates with the case manager for scheduling of annual Individual Support Plans (ISP) or special team meetings. 
  • Participants in the Individual Support Plan (ISP) meetings for pre-placement, initial, special, and annual review meetings. 
  • Supports the individual to participate in and lead their Individual Support Plan (ISP) meetings. 
  • Works with the case manager to coordinate services that support the personal outcomes of each person, utilizing natural supports wherever possible. Assists the individuals to form partnerships with established community services where available, and finds innovative ways to deliver services not readily available. 
  • Implements the ISP plan for residential services through direct observation, and reviews data and progress monthly. 
  • Ensures family, guardians, and other advocates are notified regarding changes in condition, behavior, treatment or ISP plans. 
  • Documents assessments, progress changes, and status as required.
  • Advocates on behalf of the individuals in all respects, including but not limited to, individual choice and independence in a way that enhances self-advocacy and person-centered planning. 
  • Assists the individuals in finding and selecting services and supports, both paid and unpaid natural supports. 
  • Assists in accessing supports to transition from an institutional setting, the family home, or from one provider to another under the direction of the Residential Manager. 
  • Assists with scheduling, attending and documenting medical appointments in Vista Care EHR. Communicates with the nursing team as issues arise with the individual(s).
  • Provides transportation to day services, community events, medical appointments, and other assigned transportation needs as required. 
  • Assists individuals to access community employment and ensure person centered employment planning. 
  • Initiatives a comprehensive assessment and periodic reassessment of the individual(s) needs to develop, revise and update the participants’ Individual Support Plan (ISP).
  • Ensures consents are on record. 
  • Coordinates activities regarding the implementation of the ISP
  • Communicates with the case manager when revisions to goals or supports are necessary. 
  • Knowledgeable of prevention of abuse, neglect, and misappropriation of property and self-abuse. Understand how to respond per Vista Care company policies and procedures and mandatory reporting laws. May participate in ANE investigations as needed. 
  • Will be required to administer medications to individuals and required to pass and maintain all associated training requirements
  • Other duties as assigned by direct supervisor or other leadership. 

Requirements

  • Must be at least 18 years of age
  • High school diploma, GED, or equivalent required. 
  • Bachelor’s degree in a behavioral science, social science, education, or other related field is preferred. 
  • Experience in human services, customer service, or a related field preferred
  • Must successfully complete all certification and training requirements within the assigned state(s) of operation within 90 days of hire. 
  • Valid driver’s license and clear driving record in compliance with local, state, and federal regulations and Vista Care’s best practices required
  • Prior experience in residential services and supports for persons with intellectual and developmental disabilities preferred.
  • One (1) to three (3) years experience as a Direct Services Professional (DSP) or a combination of education and experience providing equivalent knowledge required.
  • Clear background check in compliance with local, state, and federal regulations and Vista Care’s best practices
  • Demonstrated knowledge in the use of medical equipment needed to care for individuals served
  • CPR certification preferred

PHYSICAL DEMANDS AND WORKING CONDITIONS

  • Prompt and regular attendance is a requirement.
  • Ability to lift at least fifty (50) pounds on a routine basis to assist with individuals supported.
  • Ability to bend, twist, lift, reach and assist individuals supported with their movements including their ability to stand up.
  • Required to work in residential settings, apartment locations and assist in community living apartments on a regular basis.
  • Required to use their own vehicle or those owned and/or leased by Vista Care. This responsibility includes transporting persons served. A valid driver’s license and level of personal liability insurance as required by Vista Care  must remain in effect at all times.

PAY

  • $16.00 - $17.05 hourly, depending on experience.
