Eminence Home Care is looking for talented Home Care Coordinator to join our amazing team!

Eminence Home Care is seeking a Home Care Coordinator in our Glenside, PA location. As a home care scheduling coordinator, you will coordinate and manage appointments, ensuring that clients' needs and caregivers' availabilities align. You will handle scheduling conflicts, emergencies, and last-minute changes, maintaining clear communication with clients and caregivers. Additionally, you will document scheduling activities, ensure compliance with company policies, and support the delivery of high-quality care.

Responsibilities

  • Coordinate and manage client care services, ensuring the delivery of high-quality and personalized care
  • Oversee the onboarding and orientation process for new hires
  • Manage and maintain employee records, ensuring compliance with HR-related laws and regulations
  • Coordinate and schedule care staff based on client needs and staff availability
  • Monitor and manage caregiver's timecards, ensuring accurate and timely submission
  • Assess client needs and develop personalized care plans
  • Schedule and assign caregivers based on client needs and caregiver availability
  • Provide support and guidance to caregivers and clients, addressing any concerns or issues that arise
  • Monitor and evaluate the quality of care provided, making necessary adjustments as needed
  • Maintain accurate and up-to-date client records and documentation
  • Support both HR and Placement services!

Requirements

  • Minimum of 2 years of experience in HR and scheduling, preferably in the home health care industry
  • Excellent organizational and time management skills
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and caregivers
  • Ability to work collaboratively as part of a team
  • Experience with scheduling software or systems
  • Exceptional organizational and time management skills
  • Ability to handle sensitive and confidential information with discretion
