Eminence Home Care is looking for a talented Coordinator of Client and Caregiver Services to join our team!
We are looking for a highly organized, personable, and detail-oriented individual to join our team as a Coordinator of Client & Caregiver Services. This role is crucial in ensuring the seamless delivery of home care services across five counties: Philadelphia, Delaware, Bucks, Chester, and Montgomery. The ideal candidate will have excellent communication skills, enjoy working with people, and be capable of handling multiple tasks efficiently. The role involves a mix of in-field client visits and in-office responsibilities, including HR functions.
Location: Philadelphia, Delaware, Bucks, Chester, and Montgomery Counties
Salary: $45,000 - $55,000 annually + Mileage Reimbursement & Tolls
Schedule: Full-Time, Monday to Friday, 8:30 AM - 5:00 PM (In-office & In-field)
You will:
Field Responsibilities:
- Conduct initial and bi-annual client assessments to ensure proper care plans are in place.
- Travel to client homes across the five designated counties.
- Address client needs, ensuring high satisfaction and service standards.
- Maintain accurate documentation and ensure compliance with state regulations.
HR Responsibilities:
- Conduct in-person caregiver interviews and manage the onboarding process.
- Facilitate weekly caregiver orientations to ensure consistent staffing levels.
- Handle payroll submissions and promptly resolve any payroll-related issues.
- Guide caregivers through onboarding by:
- Verifying employment eligibility (I-9 documentation).
- Running background checks.
- Scheduling TB tests for caregivers.
You will have:
- High school diploma or equivalent required.
- Minimum of 2 years’ experience in home care, including conducting patient visits or reassessments.
- Strong interpersonal and communication skills.
- Attention to detail and ability to multitask effectively.
- Problem-solving abilities and proactive approach to resolving issues.
- Proficiency with Microsoft Office and basic software applications.
Other Requirements:
- Clean background check.
- Valid driver’s license and reliable transportation for client travel.
- Proof of auto insurance.
- Ability to work both in the field and office environments.
