The Intake Supervisor works with the Intake Coordinators and serves as the initial point of contact for patients and referral sources. In this role, you will make a real difference in the lives of our patients by educating consumers on our service offerings, coordinating all referrals, assigning cases to appropriate clinical resources, and ensuring follow through of the clinical process. Including direction to clinical staff and assuring continuity of care between providers and the community.

Primary Job Responsibilities:
- Receives referrals via inbound phone call and referral networks.
- Ensures that patients and referral sources are properly informed of the agency's services.
- Determines agency's ability to accept referrals via location, date, discipline, clinical requirements and patient insurance.
- Enters all necessary information into the Electronic Medical Record.
- Distributes patient assignments to clinical teams considering patient needs and staff availability and efficiency.
- Ensures patient care is initiated within regulatory and agency defined timelines.
- Communicates effectively with patients, caregivers, and referral sources regarding care initiation and changes.
- Reassigns clinical staff schedules in the event of planned and unplanned time off.
- Delivers a 5-star customer experience to all internal and external consumers.
- Accepts direction from CEO and VP of Clinical Services.
- Maintain confidentiality with regard to patient information
- Performs other duties as assigned
Requirements
- 3 + years' experience with a healthcare organization; preferably in acute care, home health or hospice environment preferred.
- RN License.
- Must be able to multi-task in a fast-paced environment and communicate effectively.
- Excellent customer service skills and proven ability to work with healthcare professionals, patients and their families responding to time-sensitive needs
- Excellent data entry skills
- Dedicated self-starter who can work both independently as well as collaboratively as part of a team of healthcare professionals.
- Proven technology skills and experience using an electronic documentation system.
- Proficient in Microsoft computer products including Word, Excel and Outlook.
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
