Social Worker

Bladen East Health and Rehab is looking for a passionate and dedicated Social Worker to join our team at our skilled nursing facility in Elizabethtown, NC. As a Social Worker, you will be responsible for providing supportive counseling and assistance to residents and their families, as well as coordinating and facilitating various programs and services to meet the social and emotional needs of our residents.

Responsibilities:

• Assessing the psychosocial needs of residents and developing appropriate care plans.
• Providing counseling, emotional support, and assistance to residents and their families.
• Coordinating and facilitating social service programs, including support groups, therapeutic activities, and community outings.
• Collaborating with the interdisciplinary team to develop and implement resident care plans.
• Advocating for the rights and well-being of residents.

Requirements:

• Current state license or certification as a Social Worker.
• Prior experience working in a healthcare or long-term care setting is preferred.
• Strong interpersonal and communication skills.
• Ability to work effectively in a team environment.
• Empathy and compassion for the elderly population.
• Knowledge of community resources and services.
