Full Job Description

OVERVIEW

The Case Manager will carry a caseload of families, individuals, or

combination of both, who have criminal legal involvement. The Case Manager, under the supervision of the Director of

Rapid Reousing, will deliver services to achieve the goals of individual or family stabilization, independent living, and

financial security.

RESPONSIBILITIES

  • Conduct initial intakes and assessments and annual assessments as required.
  • Monitor substance use services program treatment agreement/contract with residents.
  • Provide individual services to a caseload of approximately 25i ndividuals.
  • Develop initial Individual Service Plan (ISP) including relapse prevention plans with all residents on caseload.
  • Conduct quarterly ISP reviews and annual ISP update with residents as required.
  • Coordinate services with the team (Director, Social Worker, Peer Navigator, Housing Specialist, Employment Specialist, Residential Aides) in order to achieve the goals of sobriety, stabilization, independent living.
  • Refer residents to appropriate agencies and community resources for services and monitor to ensure compliance.
  • Provide bi-weekly counseling/case management, supportive service, and community building.
  • Escort residents to appointments as needed.
  • Collaborate as needed with other agencies and /or staff, including physicians, social worker/counselors, psychiatrists, pharmacy, and specialty care physicians to whom residents are referred.
  • Assist families and/or individuals with achieving goals of self-sufficiency and independent living.
  • Work with individual and/or the entire family to improve family functioning.
  • Maintain/update case notes and other data in AWARDS database as required.
  • Maintain resident’s case files to include all AWARDS and supporting documentation.
  • Provide emergency and crisis interventions.
  • Attend program meetings, agency-wide meetings, and staff training.
  • Perform program objectives in accordance with all site specific and HousingPlus protocols, policies and procedures.
  • Other tasks as needed.

Requirements

  • Minimum of Bachelor’s in Human Services or related field.
  • Experience working with individuals and families experiencing homelessness, substance use, mental illness, and criminal legal involvement.
  • Familiarity with residential social services settings and working with clients on vocational issues. Experience in facilitating groups and developing group curriculum a plus.
  • Familiarity with criminal justice reform landscape. Excellent organizational, communication, writing and computer skills. Ability to work as a multi-disciplinary team member. Some evening and weekend hours may be required.

Benefits

Salary: $45,000 - $47,000
