Sierra Health Care, Inc. is seeking a Physical Therapist Assistant (PTA) for its comprehensive Home Health Program. The PTA will provide skilled therapy services under the direction of a licensed Physical Therapist in the comfort of patients' homes. The Company operates from its main location in Truth or Consequences, where in addition to the Home Health Program, there is a professionally managed fitness center with on-site therapy services, a licensed nursing staff on-site 24/7, innovative memory enrichment programs and a vibrant social activity calendar. The Las Cruces office provides comprehensive Home Care and Hospice Programs and is staffed with experienced clinicians to ensure patients receive the highest quality of care.
Responsibilities
- Perform patients’ evaluations and obtain necessary clinical data.
- Develop and implement individualized treatment plans based on clinical observations and objectives established by a licensed Physical Therapist.
- Administer treatment programs as prescribed by the Physical Therapist.
- Educate patients and their families on proper care and use of equipment.
- Accurately document treatment plans, progress and outcomes in the electronic medical record system.
- Communicate effectively with other healthcare professionals and care teams to coordinate patient care.
- Participate in quality assurance activities and performance improvement initiatives.
Requirements
- Associate's degree from an accredited Physical Therapist Assistant Program.
- Current state license as a Physical Therapist Assistant.
- Minimum of one year of experience in a home health setting preferred.
- Basic computer skills including EMR documentation and Microsoft Office.
- Excellent communication and interpersonal skills.
- Valid driver's license and reliable transportation.
- Ability to pass a background check and drug screening.
