Occupational Therapist (OTR/L) - Home Health

Sierra Home Health & Hospice is proud to announce an excellent opportunity for a dedicated and qualified Occupational Therapist with proven skilled rehab experience to care for our home health patients in T or C and surrounding areas. Duties & Responsibilities:

• Evaluate and assess patient's level of function
• Developing an individual plan of care based on personal goals
• Implementing the treatment program through direct treatment of the patient
• Reassessing clinical signs and symptoms to determine effectiveness of program
• Instructing patient, family, and other clinical staff regarding participation in the treatment program
• Supervising and directing Certified Occupational Therapy Assistants regarding patient care

Requirements

Qualifications & Requirements:

• Bachelor's or Master's degree in occupational therapy from a university accredited by the Council of Medical Education of the American Medical Association and the American Occupational Therapy Association
• License to practice in the State of New Mexico
• Comprehensive knowledge of the principles, concepts and methodology required of an occupational therapist and its practice application in the treatment of hospice patients at home
• Excellent Communication and documentation skills
• Willingness to collaborate with an interdisciplinary team
• Valid New Mexico driver’s license and reliable, insured transportation
