As a full-service home care & hospice agency providing a broad range of services to our customers, we are currently seeking a qualified Integrated Care Coordinator to oversee our patients' care through appropriate case management. The Integrated Care Coordinator monitors the ongoing progress of patients receiving services, identifying opportunities to enhance care through additional or alternative services. This role focuses on coordinating care resources to ensure patients have access to a full spectrum of services based on their evolving needs. The coordinator collaborates with internal teams to identify and recommend adjustments to existing care plans, facilitating seamless integration between services such as Home Health, Hospice, Personal Care, or Palliative Care. 

Duties & Responsibilities:

  • Work collaboratively and professionally with internal care teams to monitor patients' needs and progress. 
  • Interact regularly with patients and their families to assess care effectiveness and explore additional services that align with evolving needs. 
  • Educate patients and families about available services and how they can improve care plans. 
  • Document and communicate updates regarding care plans, resources, and services in a timely manner. 
  • Maintain accurate patient data within designated platforms. 
  • Conduct regular check-ins with patients and families to ensure care adjustments meet their needs. 
  • Coordinate with internal teams and Primary Care Physicians to ensure timely access to services as needs change. 
  • Lead or participate in meetings to discuss patient outcomes, process improvements, and service integration. 
  • Gather and analyze metrics to evaluate the program's success and adjust processes as needed. 

Requirements

Qualifications & Requirements:

  • At least two years of combined work experience in case coordination or a related job in the healthcare field, such as a Nurse (RN or LPN), Social Worker, Mental Health Care Provider, Community Health Worker, Emergency Services, Medical Assistant, Certified Nursing Assistant, or related roles. 
  • Demonstrated problem-solving skills and the ability to manage multiple tasks. 
  • Strong collaboration and interpersonal skills; capable of managing family emotional stress and respecting individual lifestyles. 
  • Effective communication skills, both written and verbal, and proficiency in documenting care adjustments. 
  • Ability to analyze patient needs and adapt care plans accordingly. 
  • Familiarity with medical records, internet research, spreadsheets, and word processing. 
  • Current license, certification, or registration in a related healthcare field as applicable. 
  • Valid state driver’s license and availability of a car for travel. 
  • Availability – Monday through Friday, 0800-1700, with potential for on-call duties.
