Full Job Description
CareM Support Services is currently seeking an Intake Manager who is responsible for managing and overseeing the entire intake process for patients entering the home health and hospice programs. This role ensures accurate, efficient, and compliant patient admissions while utilizing technology and tools to optimize the process. The Manager collaborates with clinical teams, referral sources, and internal stakeholders to ensure a seamless patient experience and alignment with operational goals. Additionally, the Manager ensures compliance with all regulatory requirements and provides leadership to the intake team.
RESPONSIBILITIES:
- Oversee the intake process from referral to patient admission to payer authorization, ensuring smooth transitions into care.
- Lead and manage the intake team, providing coaching, development, and performance evaluations.
- Collaborate with clinical teams, operations, and administrative staff to ensure intake processes meet internal needs.
- Monitor referral pipelines, ensuring a consistent flow of admissions while addressing potential bottlenecks.
- Utilize technology and tools to enhance intake workflows and drive operational efficiency.
- Ensure compliance with state, federal, and company policies, including Medicare and Medicaid regulations.
- Maintain strong relationships with referral sources such as hospitals, physicians, and skilled nursing facilities.
- Address and resolve internal and external concerns related to intake operations.
- Monitor intake trends and metrics to identify opportunities for process improvement.
- Communicate with patients and families to ensure understanding of care options and provide support during transitions.
Requirements
- Minimum of 3 years of experience in home health, hospice, or related healthcare environments.
- Strong knowledge of Medicare, Medicaid, and private insurance regulations relevant to home health and hospice.
- Proficiency in utilizing healthcare technology, such as electronic health record (EHR) systems and referral management tools.
- Proven leadership and team-building abilities with strong interpersonal and communication skills.
- Demonstrated problem-solving and organizational skills with the ability to manage multiple priorities.
- Experience in process improvement and efficiency enhancement.
- Ability to work collaboratively with internal and external stakeholders.
