On-Call Home Health RN

Job Overview:

Our client is seeking a full-time On-Call Home Health RN to provide skilled nursing care and maintain patient safety. The ideal candidate will have experience in home health care and will demonstrate effective communication and collaboration. This role requires strong organizational skills, the ability to work independently, and a commitment to maintaining confidentiality.

Key Responsibilities:

• Provide direct or indirect skilled comprehensive nursing care to a specified (or variety) of age and diagnostic groups

• Utilize principles of the nursing process to assist in data collection, planning, and evaluation of nursing care

• Demonstrate nursing knowledge and skills in the implementation of prevention, therapeutic, and rehabilitative nursing care

• Utilize effective interviewing techniques; sets limits, and collects data based on specific role

• Communicate and collaborate effectively with health care providers, third parties, patients, and families based on specific role

• Maintain awareness of comfort and safety needs of patients

• Observe patients, records significant conditions and reactions, and notify supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents

• Respond to life-saving situations based upon nursing standards, policies, procedures, and protocol

• Use oral and written communications to convey pertinent information to members of the healthcare team in a timely manner

Schedule and Shift Details:

• 1 week on, 1 week off - 4:30p-8:30a

• Including working one summer and one winter holiday

• EMR: Home Care Home Base

• Equipment Provided: Tablet, Phone

Requirements

Qualifications:

• Active RN license in the state of NJ

• Minimum of 1 year of experience in home health care

• Strong organizational skills and ability to work independently

• Excellent communication and collaboration skills

• Commitment to maintaining confidentiality
