Full Job Description
Position Title: Health Coach
Position Type: 6 Month Grant Funded Full Time; 35 Hours/Weekly; Per Diem Work an option
Benefits Eligible:
Location: Berkeley/Bayville/Toms River, NJ
Department: Nursing
Responsibilities:
- Conduct initial consultations, including individual evaluations for new consumers
- Develop customized health care plans addressing client specific needs
- Provide consumers with education regarding substance use disorders, behavioral health, physical wellness and self-management
- Educate consumers in various treatments related to hepatitis, HIV, and STIs as appropriate
- Develop and implement self-monitoring tools for consumers in OUD, infectious disease, and co-morbid illness management
- Create a survey for clients and/or family members to complete after a health coaching session
Requirements
- Health Coach Certification approved by the National Board for Health and Wellness Coaching (NBHWC)
- Previous experience working with clients living with chronic medical and behavioral health conditions
- Maintains relevant knowledge regarding health care developments
