Acentra seeks a Clinical Evaluator (PASRR) to join our growing team.
Job Summary
As a Clinical Evaluator (PASRR) Preadmission Screening and Resident Review, you will conduct onsite assessments to facilitate appropriate placement and care for local populations in Scottsbluff, Nebraska,. Your role involves providing essential evaluations and recommendations to enhance patient well-being and quality of life. These assessments, primarily conducted in nursing homes or hospital settings, play a crucial role in determining the level and scope of care required for each patient. Your assessments will be instrumental in securing better health outcomes, providing critical support that aligns with the specific needs of each individual.
As a trusted expert, you will have the autonomy to evaluate their care needs, determinations and recommendations for their ongoing next level of care of care and helping to improve patient's well-being. This is a Hybrid PRN role that combines working from your home office with traveling to patients within your local areas. Must be open to covering primarily the Scottsbluff Nebraska area. All required travel expenses, including mileage will be fully reimbursed.
Job Responsibilities
- Conduct accurate screenings, evaluations, and determinations.
- Develop informed level of care recommendations based on thorough clinical evaluations.
- Conduct assessments in various environments, including nursing homes, hospitals, and community settings, to understand and address the diverse needs of those we serve.
- Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.
- Ensure compliance with contractual and regulatory standards while developing level of care recommendations.
- Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients.
- Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.
- Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
- Stay informed on current literature, regulations, and research pertaining to long-term care, mental illness, medical conditions, and geriatrics.
- Ensure compliance with necessary workflows to maintain quality and efficiency.
- Efficiently prioritize and execute tasks with attention to detail.

Required Qualifications, Skills, Knowledge and Experience
- Active, unrestricted licensure in Nebraska in a healthcare or related field, such as Registered Nurse, Licensed Clinical Social Worker, Licensed Clinical Psychologist, Nurse Practitioner, or other qualified Licensed Mental Health Professional.
- Minimum of 2 years' experience in assessing and diagnosing individuals with mental/psychiatric disorders, including expertise in complex differential diagnoses related to substance use, medical conditions, cognitive issues, and dementia.
- At least 1 year of experience working with individuals with intellectual disabilities or related conditions.
- Graduation from an accredited college with a degree relevant to the field.
- Working knowledge of the Preadmission Screening and Resident Review (PASRR) requirements as per Federal Regulation 42 CFR, Chapter 483, and the state PASRR rule, 471 NAC 12.
- Skilled in assessing and analyzing psychological, emotional, behavioral, and physical functioning to develop appropriate level of care recommendations based on clinical evaluations.
- Knowledge of psychiatric disorders as outlined by the latest Diagnostic and Statistical Manual in use in Nebraska.
- Understanding of standards of practice for determining Level of Care per 471 NAC 12.
- Strong familiarity with public mental health and medical programs.
- Demonstrated experience working with the geriatric population.
- Proven ability to formulate clinical diagnoses, considering psychological and behavioral factors.
- Strong communication skills, capable of effective interaction with individuals and groups via spoken, written, and electronic media.
- Skilled in using computer equiptment based software and proficiency in Microsoft Office Suites.
- Detail-oriented with the ability to prioritize and execute tasks efficiently within set timelines.

Compensation
The compensation for this role is $32.00 - $36.00 per hour.
Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
