Full Job Description
- Nurse Navigator Comprehensive Disease Management/Palliative Care
- Full Time; Monday through Friday 8am - 5pm
- St. Louis, MO and surrounding areas
- Pay Range: $70,000 - $75,000
The Comprehensive Disease Management (CDM) Nurse helps patients navigate their healthcare journey by identifying the needs, recommending resources, communicating with patient's healthcare providers and educating on disease progression and treatment/care options available. In an overwhelming time where patients and families need to make critical healthcare decisions, CDM Nurse Navigators are able to walk this journey with people so they can confidently make healthcare decisions that align with their personal goals.
Our CDM program is committed to connecting with people who would benefit from assistance in navigating the healthcare system. Phoenix's CDM program is designed to care for people who are living with chronic or life limiting illness and to provide guidance in symptom management and advanced care planning.

Responsibilities
- Connecting with patients and establishing consistent face to face encounters
- Establishing rapport with patients and initiating goals of care conversations
- Educating on the benefits and goals of the various homecare services and making appropriate referrals to those services when patient desires
- Provide assistance with advance care planning and disease specific advance directives
- Navigate patients through various care planning tasks such as coordination with specialists, disease progression conversations, and available community resources
- Completing accurate and professional documentation after each visit
- Building and maintaining strong partnerships with community healthcare partners
- Connecting and communicating at a high level with Phoenix colleagues to ensure continuity and collaboration of care.
Requirements
- Must have an active MO or Compact RN or LPN license.
- Must have active driver's license.
- Hospice or Home Health experience preferred.
