Pay Rate: $52,797

RADIAS Health is seeking a Mental Health Practitioner to join the Community Care Service Team (CCST)! This position specializes in supporting individuals who have been identified as high utilizers of emergency room/hospital services and/or have been identified as someone with high unmet needs and would benefit from additional support. The position is hybrid with the opportunity to work from home when not meeting with individuals in the community. The team uses a harm reduction and a clinic without walls approach.

Some duties include: developing, writing, implementing, evaluating and revising of treatment goals and plans in collaboration with the client and the treatment team, assisting individuals in establishing additional community supports, and communicating with natural supports. The Practitioner will coordinate services with other providers including, but not limited to, local community hospitals, Mental Health Case Management, outpatient behavioral health providers, lawyers, guardians, care coordinators, and probation officers. Practitioners on this team have the opportunity to observe diagnostic assessments and Rule 25/comprehensive assessments

Full-time Schedule: Monday - Friday 8 am - 4 pm or 9 am - 5 pm

**This position works a hybrid work schedule**

Duties & Responsibilities:

  • Work with the larger treatment team to provide services for clients. Support the development, writing, implementation, evaluation and revision of overall treatment goals and plans in collaboration with the client and the treatment team.
  • Develop an individualized crisis plan coordinated with community emergency support services.
  • Meet with clients to explain CCTS services, assess client needs, establish and implement a care plan to address those needs and arrange for resources necessary for community transition.
  • Coordinate services with other county or community agencies including but not limited to local community hospitals, mental health case management, outpatient behavioral health providers, care coordinators, and probation officers.
  • Assist in the development of comprehensive assessments, individualized treatment plans, substance abuse treatment, and other tools necessary to assist clients in the community.
  • Assist clients with all aspects of psychiatric rehabilitation and all aspects of obtaining and maintaining housing. Will assist and /or teach clients independent living skills such as cooking, shopping, cleaning, personal hygiene, transportation, and budgeting. Will assist clients with making and keeping appointments and any other tasks that are required through the client’s recovery process.
  • Attend commitment and other court hearings as needed. Complete required court reports if requested.
  • File reports with Vulnerable Adult, Child Protection, the Ombudsman’s Office, and other mandated/required reports when applicable.
  • Actively and assertively engage and reach out to clients’ family members/significant others, including landlords, employers and other treatment team members after obtaining client permission.
  • Must be able to transport clients in the community.

Requirements

  • Bachelor's degree in behavioral sciences or related field (if no degree, 30 semester credit hours in behavioral health sciences) and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders - OR-
  • Masters or other graduate degree in behavioral health sciences or related field -OR-
  • If no degree, must have 4,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders
  • Must have car, auto insurance and driver's license.
