Full Job Description
The Clinician will work with the supervisory and program staff with the Acquired Brain Injury (ABI) Division, working both with survivors of brain injuries as well as individuals in the Moving Forward Plan (MFP). These individuals are experiencing aging through the state mental health system, often having previously been provided intensive case management for diagnosed mental illness as well as substance use needs. In efforts to more appropriately assist these individuals with individualized services, who are otherwise placed in skilled nursing facilities.
The Clinician will work to develop treatment plans, behavioral guidelines and/or positive behavior plans that meet the standards established by ABI regulations. Minimize barriers in an effort to achieve the delivery of high quality evidence-based practices, programs and services, and ensure overall clinical management of program in accordance with executive leadership objectives, organizational mission, policies, practices, program funding and applicable regulatory agency guidelines.
Requirements
Essential Functions/Position Responsibilities:
- In consultation with supervisory personnel, identify areas of clinical concerns, including but not limited to: behavioral issues; social/communication deficits; independent living skills; other educational or training needs.
- Provide consultation, assessment and treatment planning in the development of pre-crisis and crisis plans, behavioral guidelines, risk assessments, positive behavior plans, medication treatment plans, etc.
- Complete thorough reviews of historical documents as needed to ensure a clear understanding of relevant history. Consult with program staff, family, and other collaterals as necessary to develop clear, concise, user-friendly treatment plans / interventions and data collection system.
- Assure all necessary program staff are trained in behavioral and other clinical interventions including data collection procedures.
- Participate in program related meetings both inter / intra agency including team meetings, risk meetings, ISP meetings, medication reviews, etc. Keep abreast of the progress and/or barriers of established plans.
- Conduct site visits to meet regularly with participants and staff to provide training, support, and relevant intervention.
- Work collaboratively with teams/ programs to develop and implement positive behavior support plans.
- Actively participate in the licensing process by ensuring all necessary behavioral guidelines and plans are up to DDS standards.
- Serve as a liaison to the ABI service system, state agencies and community partners as needed. Create and maintain open and effective lines of communication in an effort to meet contract standards and promote the mission of MHA within the community. Establish and maintain relationships with local, state, federal, and private agencies and organizations to promote programs and to identify and secure potential new alliances, partnerships and funding sources.
- Review incident reports to identify trends as well as the training needs of program staff and managers.
- Attend clinical consultations with outside providers to ensure ongoing communication. Attend intake and discharge planning meetings as appropriate.
- Provide crisis intervention and/or assessment as necessary.
- Pursue activities to enhance personal professional growth. Attend conferences, classes, address groups and represent agency. Attend/participate in statewide meetings, task forces, committees, etc. relating to area of expertise.
- Promote and integrate agency mission, values, philosophy and principles.
- Perform other similar duties as assigned by the VP of Division.
Education and Professional Experience:
- Master’s Degree in a Social Work, Applied Behavior Analysis, Psychology, Mental Health Counseling or related field.
- Minimum of 5 years of experience working with individuals with acquired brain injuries, developmental disabilities preferred, challenging behaviors or mental health issues in a clinical position.
