We are currently seeking an MDS Coordinator with PDPM experience to join our vibrant team.
Key Responsibilities:
- Current RN or LPN license in MA with previous experience in MDS/PDPM (2 years or more)
- Facilitates weekly skilled Medicare meetings and provides clear communication regarding skilled coverage criteria
- Transmits MDS to CMS and follows up to validate any errors or messages revealed on final validation reports
- Completes PDPM/MDS assessments within regulatory time frames including CAAs
- Participates and reviews invoices as indicated for skilled patients under consolidated billing
- Maintains a current PDPM schedule while setting all PDPM MDS ARDs within time frames in the MDS software program
- Completes MDS accuracy/PDPM reviews with required software/process in a timely manor
- Participate in Triple check/Compliance reviews
- Monitor skilled documentation for potential opportunities/changes
- Maintains and follows a schedule of due dates for all MDS.
- Coordinates the completion of the MDS by all disciplines.
- Reviews the MDS data for accuracy and meets with appropriate staff as needed to assure accuracy.
- Finalizes and transmits the MDSs per RAI requirements
- Complete Medicare Physician Certification and monitors compliance with CMS Medicare guidelines
- Verifies that assessments have been transmitted and approved by the State in a timely manner.
- Corrects any rejected records and prepares them for re-submission.
- Completes CAAs according to state guidelines.
- Provide weekly updates for managed care residents.
Requirements
- Previous experience in a hospital, long-term facility, or other healthcare related facility.
- Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care.
- Computer skills-Windows applications, computer experience Accu Med experience preferred.
- Previous experience in completing MDS/PDPM is required.
- Can work independently
- Flexible with schedule for month end close
- Detail oriented and able to accurately and timely complete assessments.
