Requirements
We are seeking a Licensed Practical Nurse (LPN) to join our team. Those with prior case management experience as well as familiarity with care planning, community resources, and a connection to the local community are encouraged to apply.
Whaler's Cove houses 120 residents. We do not have a memory care unit or provide advanced care (similar to nursing homes) and work with community providers to ensure that residents' needs are met even if not by our staff. LPNs have direct resident contact but are not responsible for diagnosing or treating residents. You will have the opportunity to make meaningful connections with residents and their caregivers as well as working with dedicated CNAs, all with the common goal of keeping residents in the least restrictive environment possible. Nights and weekends are rarely required, if at all. On-call is rotated in the department and commands a separate stipend.
Job Duties may include:
- Ongoing clinical assessments for existing residents as well as initial assessments for potential residents pursuing occupancy for the programs we offer
- Developing care plans and assigning tasks to staff for daily completion
- Participating in family meetings, department meetings, professional luncheons, and staff in-services/educational opportunities
- Communicating with visiting nurse agencies, physicians, pharmacies, and case management entities in support of the resident's needs
- Training CNAs and ensuring their compliance with their daily task assignments
- Data entry into our web-based software to build an Electronic Medical Record (EMR)
- Visiting with residents to provide appropriate support and resources
- Completing internal and state incident reports as well as working to maintain consistent medical records
