Our Community Care Record platform enables healthcare and community organizations to coordinate care for populations challenged with health-related social needs. Path Assist is our tech-enabled program for HRSN utilizing an evidence-based, structured intervention. Our goal is simple: increase health confidence, improve self-efficacy, and reduce inappropriate healthcare spend.
Role Overview:
Activate Care is hiring a Donation Inventory Coordinator who will play a key role in overseeing and supporting logistics while building local relationships. This is an exciting role that will help accelerate local change happening in your state to drive toward better and more equitable community health outcomes.

- Research and cultivate relationships with individual and corporate donors and community agencies for the solicitation of in-kind goods
- Communicate with potential donors regarding items to be donated, as well as planned delivery coordination of the item donation
- Exercise discretion regarding acceptance and/or disposition of donated goods and maintain donor relationships through attentive customer service
- Work with clients to ensure the proper processing of donations through the appropriate software & tracking tools
- Complete donation paperwork for donors
- Monitor the donation rooms to ensure organization and cleanliness
- Partner with other locations, and help facilitate logistics related to the delivery of extra donations to other shelter sites or partner agencies
- Have a passion for helping others with whole-person wellness
- Experience working with individuals and families to make sure they have the support and resources needed to meet their social and health needs
- Have attention to detail, and a desire to arrange, organize and help others plan ahead

Responsibilities:
- Research and develop community partnerships with individuals and organizations to help acquire corporate donors for the solicitation of in-kind goods
- Respond to inquiries, phone calls, and emails in a timely manner
- Complete all documentation required to track donation receipts and distribution of goods
- Provide regular updates and reports to site management
- Follow standardized processes and internal procedures
- Sympathetic to individuals and their family's unique dynamics to help arrange appointments based on their unique needs health, wellness, and goals.
- Arrange, schedule, guide, and educate clients on transportation options for individuals and families
- Maintain accurate records of interaction with clients and acceptance of any donations
- Other duties as assigned
Requirements
Qualifications & Skills:
- Experience working directly with nonprofits, healthcare organizations, social service providers, faith-based groups, or government agencies that address social determinants of health
- Strong with communication skills, empathy, understanding and problem-solving abilities
- Capable of working independently with minimum supervision
- Ability to perform physical work, including lifting to 50lbs unaided is required.
- Minimum two years’ experience working in Human Services highly preferred
- Ability to juggle multiple tasks, set priorities, effectively, manage time, and meet deadlines
- Strong accuracy and attention to detail
- Excellent written and verbal skills in the English language and Haitian Creole are required
- Proficiency in using Microsoft Suite, the Internet, and other software applications
- Cultural humility: You are able to understand, communicate and work effectively and respectfully across demographical, socioeconomic, language, and all other constituents that represent diverse cultures of communities.
- A valid driver's license in the state of Massachusetts is highly preferred
- Additional language skills are a plus!
