We are currently seeking an Admissions Liaison with a strong clinical background to join our vibrant team. Your Role:

As an Admissions Liaison, your role begins with the strategic coordination with hospitals, healthcare providers, and referral sources to ensure a seamless transition for prospective residents into our community. You will manage communications and processes between these external partners and our facility, laying the groundwork for a smooth and supportive admissions process. You will also be a first point of contact for families and prospective residents, guiding them with empathy and professionalism through the intricacies of admission. Your efforts are pivotal in ensuring that every new resident experiences a welcoming transition, fully supported by a collaborative network of care that extends from their previous healthcare setting into our community. This role demands not only a strong clinical background but also exceptional coordination skills and a deep commitment to providing compassionate support during a crucial time in our residents' lives.

Key Responsibilities:

Coordinate Admissions: Work closely with hospitals, healthcare providers, and referral sources to facilitate seamless transitions for new residents, ensuring all necessary medical information and documentation are accurately transferred and received.

Assess Resident Needs: Conduct initial assessments in collaboration with clinical teams to determine the specific care needs and suitability of each prospective resident, ensuring a personalized and appropriate care plan can be developed upon admission.

Facilitate Smooth Transitions: Liaise between clinical, administrative teams, and external partners to streamline the admissions process, minimizing stress for residents and their families during the transition.

Compliance and Advocacy: Ensure adherence to all relevant health care regulations, policies, and best practices throughout the admissions process, acting as a knowledgeable advocate and compassionate resource for families and new residents, guiding them through the procedural landscape with empathy and support.

Build and Maintain Relationships: Establish and maintain positive, collaborative relationships with referral partners, fostering a network of support that enhances the quality and continuity of care for incoming residents.

Requirements

  • A strong clinical background (nursing or relevant healthcare experience preferred; licensure as an LPN or RN is not required).
  • Experience with PCC, CarePort, and general familiarity with patient portal systems
  • Excellent communication and interpersonal skills.
  • Compassion and dedication to providing exceptional care and support.
  • Ability to work flexibly and collaboratively within a multidisciplinary team.
